New Enrollment Checklist

The following items are requested upon enrollment:

____ Copy of enrollment forms - Demographic Information Sheet (See Enrollment forms to download and complete)

____ Copy of Birth Cerficate

____ Copy of Social Security Card

____ Copy of Parent/Guardian Driver's License

____ Complete Passort (if applicable)

____ Current Tennessee Innumization Record

____ Custodial/Residential Parent Documentation

____ Proof of Residency

Please note: All students experiencing homelessness shall be immediately enrolled, even if they are unable to provide enrollment documents listed above. If your family is in a temporary living situation, please contact Penny Monks at pmonks@gcboe.us for assistance with enrollment and available support services.
